РОЛЬ МАКРОФАГОВ В РЕГЕНЕРАЦИИ КОСТНОЙ ТКАНИ: МЕХАНИЗМЫ, ОСОБЕННОСТИ ОСТЕОИММУННОГО ОТВЕТА НА МЕТАЛЛОКОНСТРУКЦИИ И ПЕРСПЕКТИВЫ БИОИНЖЕНЕРИИ

Резюме. В обзоре систематизированы современные данные о клеточно-молекулярных механизмах участия макрофагов в регенерации костной ткани. Рассмотрены особенности макрофагального ответа при имплантации металлоконструкций и оценен трансляционный потенциал иммуномодулирующих биоматериалов для оптимизации исходов остеосинтеза. Показано, что макрофаги являются центральными клеточными интеграторами остеоиммунного микроокружения, координируя воспалительную, ангиогенную и остеогенную фазы репарации. Эффективная активация остеорегенерации в ответ на травму (механическое повреждение), требует создания строго упорядоченной во времени межклеточной коммуникации макрофагов с нейтрофилами, Treg-лимфоцитами и мезенхимальными стромальными клетками. Инициация реакции на инородное тело при имплантации металлоконструкций смещает баланс в сторону персистирующего воспаления и RANKL-опосредованного остеокластогенеза. Разработка остеоиммуномодулирующих биоматериалов и биоактивных покрытий, программирующих макрофагальный фенотип in situ, открывает принципиально новые возможности в лечении пациентов с костной патологией, прежде всего в травматологии и ортопедии.
