Факторы, влияющие на комплаентность к терапии антиконвульсантами при эпилепсии (Обзор литературы)

Резюме. Нарушение комплаентности к терапии антиэпилептическими препаратами наблюдается от 30 до 40% страдающих эпилепсией, что обуславливает неблагоприятные исходы заболевания. Несоблюдение режима лечения приводит к увеличению частоты эпилептических приступов, что влечет за собой увеличение прямых и непрямых расходов здравоохранения. Неприверженность лечению антиконвульсантами повышает заболеваемость и смертность. В настоящее время идентифицировано порядка 250 факторов, обусловливающих отношение больных к соблюдению режима терапии. Выделено 4 основных группы: факторы, связанные с терапией (частота приема, количество препаратов, побочные эффекты); факторы, связанные с больным, с его личностью (образованием, отношением к болезни и др.) и заболеванием (тяжестью течения, особенностями приступов и др.); факторы, связанные со взаимодействием «врач-пациент», профессионализмом врача и социально-экономическими факторами (окружением больного, профессиональной деятельностью, уровнем доступности специализированной помощи и лекарственных средств). Дальнейшее изучение причин низкой приверженности к терапии, в т.ч. в российской популяции пациентов, позволит внести необходимые коррективы в систему оказания специализированной медицинской помощи больным эпилепсией.
